This form is for National Assistance Card holders who need to update or replace their Card.

How it works

A National Assistance Card Team member will contact you to confirm details
You will receive an email with a link to log back into your application and finalise your request.

Please note: If you are only updating the information linked to the QR code on your Card, no cost applies. If you require a Card to be printed and posted to you, a cost of $22.00 applies.
