Applying for a National Assistance Card is done online through a secure portal. You can complete the application at your own pace, save your progress, and come back at any time.

The National Assistance Card requires you to have supporting documentation from a medical or allied health professional that confirms your disability. Please check that you have these document/s available before you begin your application.
The documents must be from a medical or allied health professional and need to include the applicant's name and disability or health condition diagnosis. The documents do not need to go into detail.
We accept letters, reports, and certificates from the following professionals:
A scan or photo of the document can be uploaded directly to the application.

The online application can take approximately 30-45 minutes to complete. You may find it helpful to apply with a support person. If you need help at any stage, you can also call us on our free phone line 1300 680 045 and we can support you through the process.
The online application includes 7 steps:
1. Share some personal details
You will enter some basic personal information.
2. Build the Card (Front)
You will choose what appears on the front of your Card.
3. Build the Card (Back)
You will choose what appears on the back of your Card.
4. Build the QR code (optional)
You will choose what additional information is linked to your QR code.
5. Upload eligibility documentation
You will upload documents from a medical or allied health professional that confirm your disability.
6. Review the application
You will check that all information is correct before submitting.
7. Payment
You will complete payment to finalise your application. (A new Card costs $44.)
A fee waiver is available for people experiencing social or financial hardship.
You can save your application and close the portal at any time. You can return later to continue where you left off.
When you are ready, click the Apply Now button. This will take you to a secure online portal where you will create your application and work through six simple steps. Once you submit your application, we will review it and contact you if we need any further information.
